Nutrition and Diabetes Center
 

Diabetes is a disease that can have serious effects on many parts of the body,

sometimes permanently. If left untreated, diabetes can lead to serious problems. Scientific research has proven that controlling blood glucose levels can reduce the risk of developing long-term complications of diabetes.
 

The Nutrition and Diabetes Center is an accredited program which meets national standards for diabetes self-management education. We offer periodic assessments of patient progress in reducing A1C levels and improving self management skills. Our mission is to give you the tools you need to control your blood glucose levels over the long term to enhance your quality of life.

Services

Adult Type 1 Toolkit
  • Individual assessments
  • Support and education for:
    • adult
    • Type 1
    • Type 2
    • Gestational diabetes
  • Group education sessions (physician order/referral necessary)
  • Support group
  • Individualized nutritional counseling (physician order/referral necessary)
  • Treatment plan collaboration and follow-up with participant and physician
  • Gestational diabetes education



Outpatient Services

The Outpatient Diabetes Self-Management Education Program (DSME) is based on current national standards for diabetes self-management and is certified by the American Diabetes Association and may be insurance reimbursed.

Working with you and your doctor, our team creates a customized care plan just for you that includes a wide range of topics such as:

  • Blood glucose/ A1C monitoring
  • Behavior change and goal setting
  • Oral medication
  • Insulin self-management skills
  • Insulin Pump Training/Support
  • Nutrition counseling & meal planning
  • Support for you and your family
  • Exercise guidelines & stress management
  • Diabetes counseling during pregnancy
  • Complications prevention

 

Preparing for Your Visit

Schedule your Diabetes Education and/or nutrition appointments through our Central Scheduling department at 410.427.5500.

On your appointment day, please bring the following items:

  • list of current medications and nutritional supplements
  • medical insurance information
  • blood glucose meter
  • food records you have been keeping

Inpatient Services

Management of your blood glucose level is perhaps even more important when you are in the hospital. Studies have shown that keeping blood glucose levels near normal while hospitalized can help decrease length of stay, prevent complications and improve outcomes overall.

Our experienced Inpatient staff will work with your doctors and nurses in the hospital to give you the highest quality care. This includes helping to manage your blood glucose levels, and teaching you basic skills to manage your diabetes at home until you can return to the outpatient program for more advanced instructions.

Diabetes Education Class with the dietician is just part of the diabetes education program we offer at St. Joseph Medical Center. 

Free Support Groups

Diabetes Info Exchange: offers additional diabetes management education on many topics for Type 2 diabetics.
Type 1 Toolbox: offers people with Type 1 diabetes, age 18 and older, the opportunity to meet and talk to other type 1s who are living with diabetes.
